Higher Grade Qualified Social Worker - Continuing Healthcare/117 Team

Do you have excellent assessment skills with the ability to cope with a case workload efficiently and effectively?

We are currently looking for Higher Grade Qualified Social Worker. You will be working within the Integrated Continuing Healthcare (CHC) team supporting the completion of Continuing Healthcare assessments within the community and Jubilee House (inpatient unit for CHC assessments and end of life care). You will also be assessing individual needs and working in a person centred way to support timely and appropriate discharges from Jubilee House.

With a qualification in social work as well as being HCPC registered you will have substantial experience as a qualified social worker.
